Michael’s Tree and Loader Service, LLC keeps a record of its disaster recovery subcontractors’ contact information and their capabilities. If you would please fill out all of the information requested, as well as send us current copies of your workers compensation and liability insurance certificates and a completed W-9 we would appreciate it.
Equipment
Please list all equipment related to disaster event work that you OWN or lease on a regular basis and operate with your employees on a separate sheet. If you have 3 bucket trucks you do not have to list each individually, just note the quantity. Please include bucket trucks, hauling equipment, skid steers, excavators, loaders, grinders and chippers, as well as any staff with important certifications, such as line clearance certification and certified arborists.
Please list all equipment related to disaster event work that you could provide through subcontractors. All subcontractors need to have workers compensation and liability insurance as well. We are not asking for your subcontractor’s names, just the types and quantity of equipment they can provide.
Make, Model and Year of equipment is not necessary. You can list equipment as a 55 foot working height bucket or a 55 cy knuckle boom or a 700 hp tub grinder would be fine.
